MEETING PROGRAM REQUIREMENTS
Students who maintain an active status in their program without interruption are responsible for meeting all current requirements, including requirements that are added or revised since the student first entered the program. Students who interrupt their studies by officially withdrawing from the university or by losing their active student status are held responsible for their program requirements in force at the time they are readmitted or are reinstated into the program.

The program is designed to be completed in nine quarters. In the event a student cannot complete the program in consecutive quarters, they will have 18 quarters from their initial start date to complete the Bachelors of Science degree in Health Information Management. If re-entry into the program extends the completion date beyond the 18 quarter limit, students will be required to retake the applicable courses that fall outside of the 18 quarter time span.

MINIMUM ACADEMIC PERFORMANCE
In order for students to achieve academic success, it is imperative that students earn grades that meet at least the minimum requirement of the program. Students must maintain at least a grade of “C” in all courses or the course must be repeated. A withdraw from a course is also considered an unsuccessful attempt. When the student decides to repeat the course, they must contact the HIM Staff Assistant to initiate a grade replacement form‚ according to the Grade Replacement Policy. Consequently, if a student receives two unsuccessful attempts (grade of D, F, or W) the student will be suspended from the program for a period of one year. After one year, the student may reapply for admission to the University and the HIM program. If a student returns after an absence of more than one year, faculty will advise the student on repeating previously completed courses based on changes in the curriculum or course content. After students have been suspended from the program for one year, they may be offered re-admission under the following conditions: Must repeat all courses in which they earned “D”; If they earn one more grade that is below “C”, they will be dismissed from the program with no opportunity for reinstatement.

GRADUATION
All undergraduate and graduate students must submit their graduation applications through the Online Graduation application. The colleges will not accept paper applications. Be sure to apply for degree by the deadline for the term in which you wish to graduate. Check the Master Schedule of Opening Dates and Closing Deadlines for details. Late applications will not be accepted. The Online Graduation Application will collect a $35.00 fee by credit card as part of the application process. PROGRAM TERMINATION
If a student wishes to terminate their participation in a program they may do so by contacting the Health Information Management Executive Staff Assistant at 513-558-2413.

Complete withdrawal (i.e., withdrawal from all classes for which you are enrolled that term) may also be initiated by the student in writing on a complete withdrawal form found in the college offices.

Actions which are not considered as official notification of withdrawal are:

  • Assuming classes will be canceled for non-payment.
  • Failure to attend class.
  • Giving notice to an instructor.
  • Stopping payment on a check used to pay fees.
  • Crossing out class(es) on the schedule/bill and returning only partial payment to the cashier.

The effective date of a withdrawal to be used in determining refunds is the date that the withdrawal is submitted to the Office of the Registrar for processing. Refunds are processed automatically upon receipt of the withdrawal and are mailed from the Office of Student Accounts. To receive 100 percent refund, a student must drop a class or classes by the eighth calendar day of the quarter regardless of the instructional fee date or the payment due date on the bill.
